Packaging of Specimens Test Requisition Forms

Use a ‘Zip lock bag' with double pockets for packaging specimens and test requisition forms (TRF).
Fold the test requisition form with the patient's name and bar code facing out.
Place the specimen(s) wrapped in absorbent paper, in the rear pouch (printed side) and the requisition form in the front pouch (un-printed side) with the bar code facing out.
Seal the Zip Lock bag. This will prevent the sample from contaminating the TRF in cases of accidental leakage.
Transport the specimen bag at specified temperatures as per the reference guide.
