Packaging of Specimens Test Requisition Forms
- Use a ‘Zip lock bag' with double pockets for packaging specimens and test requisition forms (TRF).
- Fold the test requisition form with the patient's name and bar code facing out.
- Place the specimen(s) wrapped in absorbent paper, in the rear pouch (printed side) and the requisition form in the front pouch (un-printed side) with the bar code facing out.
- Seal the Zip Lock bag. This will prevent the sample from contaminating the TRF in cases of accidental leakage.
- Transport the specimen bag at specified temperatures as per the reference guide.
